Shiloh Manor's most recent state inspection, in March 2026, ended with no deficiencies cited — the strongest result an Oklahoma facility can get. That visit also included a complaint investigation covering seven allegations, among them abuse, privacy, choice of doctor and pharmacy, and special diets; after observing residents, meals, rooms and medications and reviewing charts, incident reports and resident council minutes, the surveyor concluded the facility was in compliance with regulations. The earlier January 2025 inspection did find one problem — the yearly electrical safety inspection had been overlooked for 2024 — and the facility had it completed the next day, with the state confirming compliance in March 2025.

— The January 2025 relicensure survey did cite one deficiency under the plumbing and electrical rule for failing to complete a yearly electrical inspection; the last one on file was from 08/17/23. It was graded at the state's widest scope - potential for more than minimal harm reaching across the community, though no actual harm was identified. The facility corrected it on 1/16/2025 and OSDH confirmed compliance on an offsite/paper revisit dated 03/07/25.
