Shiloh Manor's most recent state inspection, in March 2026, ended with no deficiencies cited — the strongest result an Oklahoma facility can get. That visit also included a complaint investigation covering seven allegations, among them abuse, privacy, choice of doctor and pharmacy, and special diets; after observing residents, meals, rooms and medications and reviewing charts, incident reports and resident council minutes, the surveyor concluded the facility was in compliance with regulations. The earlier January 2025 inspection did find one problem — the yearly electrical safety inspection had been overlooked for 2024 — and the facility had it completed the next day, with the state confirming compliance in March 2025.
