What state inspectors found

Superior Care Homes of Oklahoma is a small five-bed home that had one state inspection in the past two years, in April 2025. Inspectors cited two problems. The medication room was found unattended with the door standing open and no lock on it — the surveyor noted this violated the home's own written policy requiring medications to be stored in a locked area accessible only to authorized personnel, and the citation was scoped as having the potential to affect all residents. Second, one of three resident records checked was missing its required annual comprehensive assessment, for a resident admitted in 2023 with dementia; the consulting nurse told the surveyor assessments were done "Only on admission." The other two assessments reviewed were current. Both findings were corrected, and after an offsite paper revisit on June 5, 2025 the state confirmed the home back in compliance effective May 31, 2025.
