The community creates "a warm, welcoming environment where everyone feels like family," with staff collaborating with residents, families, and healthcare providers to develop personalized senior care plans in a pet-friendly setting. Assisted living provides 24/7 help with bathing, dressing, grooming, and mobility, medication management, and nurse coordination, while person-centered memory care -- backed by National Institute for Dementia Education (NIDE) certification and an emphasized high staff-to-resident ratio -- focuses on "uncovering each resident's unique capabilities."

State Inspection Record

Oklahoma inspects every assisted living and memory care community on a regular schedule, plus any time a complaint comes in. Inspectors are state employees, not part of this site — what follows is what they found, in their own words.

What state inspectors found

Teal Creek has been inspected four times in the last two years, and three of those inspections ended with no deficiencies cited at all — including a February 2026 investigation of complaints that alleged abuse, short staffing, and unanswered call lights. Across those visits, surveyors personally watched staff giving medications per doctors' orders, helping residents at meals, and answering call lights, and described residents as clean and well groomed. The exception is the November 2025 relicensure survey, which found real problems. The state cited the kitchen at a 'widespread' severity level for dirty food-prep and dry-storage floors, unlabeled and undated food and one expired container of chicken salad in the refrigerators, and use of unpasteurized shell eggs served fried, boiled or scrambled. It also cited three admission assessments completed a few days late, and — most relevant to daily care — found that the four residents on pureed diets were served the same breakfast (oatmeal, juice and milk) every day with no choices, that lunch items on the posted menu such as garlic bread and dessert were not provided to them, and that food was thinned with tap water rather than milk or gravy, so nutritional content was not maintained. The director of nurses told the surveyor the center had no written policies for kitchen cleaning, food labeling, unpasteurized eggs, or pureed foods. The facility submitted a correction plan the state accepted, and a complaint investigation about two and a half months later found nothing — but the bundle contains no revisit letter verifying those specific corrections, so families should ask to see the kitchen and ask specifically how pureed and special diets are planned and served today.

  • Three of the four state inspections in the last two years ended with no deficiencies at all — including a February 2026 visit that looked into three separate complaints. 02-04-2026
  • In February 2026 the state investigated complaints alleging abuse, too few staff, a broken call light system, and an unclean environment. Inspectors spent three days on site and closed the investigation without citing the facility on any of it. 02-04-2026
  • During that February 2026 visit, surveyors watched residents actually receiving hands-on help at mealtimes, taking part in therapy and activities, and observed how long call lights took to be answered. 02-04-2026
  • Even in the November 2025 inspection that did result in citations, the three care-related complaint allegations investigated at the same time — care not following the plan of care, inadequate supervision to prevent accidents, and incontinence care — were each marked as not resulting in any citation. 11-20-2025

  • After an August 2024 complaint about hot water, the surveyor checked resident rooms and a community bathroom/shower, confirmed hot water was available in the community shower, reviewed maintenance records and plumber invoices, interviewed the maintenance director about the corrective measures taken, and cited no deficiency. 08-30-2024
  • On that August 2024 walk-through, the residents the surveyor saw were clean and well groomed. 08-30-2024

What the state also noted

  • OSDH accepted the facility's plan of correction (letters dated 12/22/2025 and 12/23/2025) with a stated compliance date of February 9, 2026, but the bundle contains no revisit letter confirming the corrections were verified on site. The clean February 2026 survey was a complaint investigation, not a formal revisit of those citations.
  • The 'no deficiencies' language in the February 2026 cover letter refers to the facility as an 'Adult Day Care facility,' which appears to be a clerical error in the letter template; the attached state form correctly identifies it as Teal Creek Assisted Living & Memory Care. The 12/22/2025 letter also contains a typo giving the compliance date as 'February 9, 2025'; the 12/23/2025 corrected notice states February 9, 2026.
  • The August 2024 complaint alleged the facility had no hot water. No deficiency was cited, but the investigation record shows there had been a real hot water shortage: the surveyor reviewed plumber invoices and hot-water maintenance logs and interviewed the maintenance director about corrective measures and 'what areas had been affected.' The report confirms hot water was available in the community bathroom/shower; it states resident rooms were checked but does not record the result.
  • The November 20, 2025 relicensure survey cited four deficiencies: a food storage, preparation and service violation rated widespread, across the community, for unclean kitchen floors, unlabeled and undated food, and an expired chicken salad container; a second food-safety violation for unpasteurized shell eggs used to serve residents fried, boiled or scrambled eggs; an assessment violation for three admission assessments completed after the required deadline; and a violation for pureed diets. OSDH's cover letter stated these deficiencies 'represented the potential for more than minimal harm.'
  • The pureed-diet citation is a care-quality finding, not just paperwork. The center failed both to follow the daily menu for pureed foods across 3 of 3 meals observed AND to maintain the nutritional content of pureed items: the cook blended roast beef and brussel sprouts with tap water, pureed-diet residents received oatmeal, juice and milk every breakfast with no options ('pureed diets all get the same thing'), and menu items such as garlic bread and dessert were omitted from their plates.
  • The kitchen citation was the widest-scope finding in the record — the surveyor documented debris, pooled brown liquid, and a black substance on food-prep and dry-storage floors, and the director of nurses stated the center had no written kitchen, food-labeling, unpasteurized-egg, or pureed-food policies and simply 'followed state regulations.'
