What state inspectors found

The Arbors was inspected in October 2025 in a combined relicensure survey and investigation of three complaints, with surveyors on site unannounced across five days. On the abuse and medication allegations the state found no deficient practice: surveyors watched medications being administered, saw professional interactions between staff and residents, and no resident complained to them about staff, about medication being withheld, or about not getting help with daily activities. Residents also confirmed they chose their own outside providers. This was not a clean survey, however - five deficiencies were cited. All three resident bathrooms tested had bathing water above the 115-degree limit, one at 132 to 135 degrees. None of the six sampled residents had documentation that a nurse or pharmacist reviewed their medications monthly, and none of six sampled assessments were signed by the resident or their representative. An involuntary-discharge notice was missing required components, including the date the resident had to leave and how to request a hearing. Most seriously, the center was cited for failing to assess and monitor skin integrity for a resident with radiation burns who was undergoing radiation and chemotherapy; a progress note shows twice-daily cream orders were reduced to once daily because the center had no nurse on site around the clock and home health could visit only once a day. That resident moved out in September 2024. The center had a plumber adjust water temperatures two days after the inspection and submitted a correction plan the state accepted, but this bundle contains no revisit letter confirming the fixes were verified on site.

  • Complaints alleging that residents were verbally or psychosocially abused were investigated by the state, and surveyors found no deficient practice related to resident abuse or to pharmaceutical services. 10-22-2025
  • When the surveyors were on site, no resident told them staff were withholding medication or failing to help them with daily activities. 10-22-2025
  • Residents themselves told the surveyors they got their medications when they needed them, and staff confirmed medications were given as the physician ordered. 10-22-2025

What the state also noted

  • The plan-of-correction letter records a prospective compliance date - deficiencies 'will be corrected' by 11/17/2025 - and the bundle contains no revisit letter verifying corrections were completed.
  • The skin-integrity citation was a failure to ASSESS AND MONITOR skin integrity, not merely a documentation lapse. The resident had radiation burns to the lower abdomen and perineal area, and the 08/29/24 progress note records that twice-daily wound-care creams were cut to once daily because the center had no nurses in the building 24/7 and home health could only come once daily. Requested hospital records were never provided to the surveyor. The resident moved out 09/08/24.
  • Five deficiencies were cited, so this was not a clean survey: involuntary termination notice (isolated, one or a few residents), assessment interview/signature, hot water over 115F (a pattern, several residents), monthly medication review (a pattern, several residents), and skin integrity (a pattern, several residents).
  • The neglect portion of the abuse/neglect allegation was NOT cleared. The investigative reports state only that there was no deficient practice 'related to resident abuse or pharmaceutical services'; the wound-assessment and involuntary-discharge allegations in complaint OK00073572 both resulted in citations - one for skin integrity, one for the involuntary termination notice.
  • All three sampled resident bathrooms exceeded the hot-water limit (123-127F, 132-135F, and 124F). Water at 132-135F is a scald risk for a frail adult, not a paperwork issue.
  • Only one survey from the last 24 months is included, so there is no earlier survey in this bundle to compare against for trends.
