This 35-bed community in Enid had one inspection in the last two years, in November 2025, combining a relicensure survey with a complaint investigation, and the state cited two deficiencies - so the record is not spotless. One of them is more than paperwork: a resident prescribed Lyrica three times a day for nerve pain had four scheduled doses recorded as not given in June 2025 (three with no reason written down, one noted as held for behaviors), and the facility's own care coordinator told the surveyor the medication should not have been held for a resident with a pain diagnosis and that the record showed no other pain medication was given instead.
