State inspectors spent three days at The Courtyard at Magnolia Creek in November 2025, combining a full relicensure survey with three complaint investigations, and cited no deficiencies. The complaints were serious ones, including allegations of abuse, misappropriation of property, and unsafe transport, and none of them resulted in a citation. Surveyors watched staff help residents through meals, medication times, and activities and described the interactions as dignified and respectful, and they reviewed clinical records, care plans, incident reports, and staff training. They also interviewed residents and family members directly about the care being given.
