📞Often nothing to worry about. The state’s findings are summarized below — call Paul if you want help judging whether any of them would matter for your loved one.
Based on Oklahoma state (OSDH) survey history — assisted living and memory care have no federal star rating.
The state cited an immediate jeopardy — its most serious level — in this community’s recent record. Read the findings below before you call, and ask the administrator what has changed since.
3 inspections in the last two years · 1 with no deficiencies cited · 2 with findings
State inspectors visited The Gardens at Quail Springs three times in the last two years and looked into eight separate complaints. Seven of those complaints - covering abuse, daily personal care, food temperature, call lights, laundry, cleanliness and infection control - were investigated and closed with no citation, and surveyors specifically noted seeing staff engaging with memory care residents during activities. However, families should know that one complaint was substantiated: in July 2025 the state cited the center at the most serious level (immediate jeopardy) after two memory care residents left the building unsupervised on June 21, 2025 and were found by police in a nearby hospital parking lot. Both residents were unharmed, the center corrected the problem within a day, and a follow-up state visit on August 25, 2025 confirmed all deficiencies were cleared.
Summarized from Oklahoma State Department of Health survey documents. Inspections are a snapshot of specific days and sample a small number of residents.
