The third visit, a full relicensure survey in August 2025, was very different: inspectors cited multiple problems, including a finding of neglect that harmed a resident who lay on the floor for nearly two hours after a fall, plus kitchen sanitation, missing care plans, an incomplete quality-assurance program, a direct-care aide with no documented CPR or abuse training, and failures to notify families of incidents.
