The Health Center at Concordia's most recent inspection, in October 2025, covered a full relicensure review plus three complaints and ended with no deficiencies cited — surveyors saw staff answering call lights promptly and helping residents, and the residents they interviewed said they felt safe. That said, the record is not spotless: a December 2024 complaint survey cited the center for not giving medications as the physician ordered, including a case where a resident received a double dose of an antibiotic for five days and had to be sent to the emergency room. A February 2025 follow-up visit found a repeat problem with medication documentation, and the state did not confirm the facility back in full compliance until April 2025. If you are considering this community, it is fair to ask the administrator directly how medication administration is checked and audited today.
