What state inspectors found

The Heaven House is a small five-bed home, and its single recent state inspection (October 2025) turned up paperwork problems rather than problems with hands-on care. Inspectors cited an annual assessment that was overdue, an assessment that had not been updated to show a resident now needs a mechanical lift, and a care accommodation plan that was being reviewed yearly instead of quarterly. On that same visit the surveyor watched two aides use the lift together to move that resident safely, which is exactly what the resident's doctor had ordered. The home's first correction plan was rejected by the state and had to be rewritten; the amended plan was accepted and a December 2025 review found the home back in substantial compliance.
