The Heaven House is a small five-bed home, and the state's one inspection in the past two years — a licensure survey on October 22, 2025 — cited four problems. A nurse aide was giving medications with a certificate that had expired the month before; a resident using oxygen had no physician's order for it and no 'oxygen in use' sign posted in the room; and abuse-prevention training records were missing from four of the five staff files reviewed, with the director of operations saying, 'We do it, we are just not organized here.' The home submitted a correction plan the state accepted, and a follow-up review on December 4, 2025 found it back in substantial compliance.
