The Heaven House operates locally owned and operated residential care homes in the Oklahoma City area, serving seniors for over 14 years. Each home is set in a private neighborhood and houses just five residents with a 5:2 resident-to-staff ratio, providing 24-hour care for individuals needing assistance with the activities of daily living in a genuine home setting rather than an institutional facility.
Certified, well-experienced caregivers tailor service to each resident, providing help with bathing, dressing, toileting, and grooming, medication administration, and three home-prepared, nutritionally balanced meals served family-style. An on-staff physician is available for consultations, a registered nurse visits weekly, and residents enjoy Bible study, crafts, games, monthly music and art therapy, weekly beautician visits, and twice-weekly personal trainer sessions.

This is a very small home — five licensed beds, with four residents living there at the time of the inspection. Oklahoma State Department of Health surveyors conducted the relicensure inspection on October 28, 2025 and cited no deficiencies. That is the only survey document on file for the past two years, and it contains no surveyor notes about daily care, staffing, medications, or the building. Read that clean result alongside the home's history rather than on its own: the file carries a 'Watch' status because of an earlier actual-harm neglect finding — failure to monitor and treat a worsening pressure wound (February 2023) — and a double-dose medication error that reached a resident (April 2024). Those underlying survey documents are not included in the material reviewed, so their details could not be independently verified here, but they are the reason one recent clean inspection should not settle the question. If you are considering this home, visit in person and ask specifically about wound care, skin checks, medication handling and double-check procedures, and overnight staffing.
