The Legend at Jefferson's Garden had one state survey in this period - a routine licensing inspection on October 9, 2025, not one triggered by a complaint. Inspectors cited four problems: prepared food in the refrigerator that was not labeled or dated, two resident assessments that were filled out inaccurately, three assessments missing the required nurse or physician signature, and two washing machines running at 112 degrees when 160 degrees is required to properly sanitize laundry. The community submitted a correction plan the state accepted, and on November 12, 2025 the state closed out the citations as corrected effective October 10, 2025, returning the community to substantial compliance. Worth knowing: that follow-up was an offsite paper revisit, meaning the state reviewed submitted documentation rather than returning to the building to see the fixes in place. Because the laundry temperature finding was scored as having the potential to affect every resident, it is reasonable to ask the administrator to show you the current laundry temperature logs and to explain how assessment accuracy and nurse sign-off are being checked month to month.
