State Inspection Record

Oklahoma inspects every assisted living and memory care community on a regular schedule, plus any time a complaint comes in. Inspectors are state employees, not part of this site — what follows is what they found, in their own words.

Built by Paul Swales, a senior placement advisor, from Oklahoma State Department of Health inspection records — not from reviews. Assisted living and memory care have no federal star rating.

What state inspectors found

1 inspection in the last two years · 1 with findings

The Neighborhoods at Baptist Village of Okmulgee had one state inspection in the last two years, a three-day October 2025 relicensure survey that also investigated a complaint. The complaint alleged a pest problem and financial exploitation of residents; the surveyor toured the building repeatedly and saw no pests or bedbugs, residents said they had not seen any either, and residents said they had never been asked for or given money or gifts to staff - neither allegation resulted in a citation. The surveyor also noted staff assisting residents with daily activities in a pleasant manner. One deficiency was cited, in the kitchen: while dishes were being washed, no sanitizer registered in either the three-compartment sink or the dish machine, the dish machine's sanitizer container was empty, the required daily sanitizer check had not been done that day, and the facility could not produce a policy on sanitizer use and monitoring. In its plan of correction the facility said it replaced the sanitizer drum the same day, rewashed the affected cookware and utensils, retrained the dining team, wrote a new procedure and added ongoing monitoring, and that nursing watched the 17 residents who eat kitchen-prepared meals for gastrointestinal symptoms with none observed. The state confirmed the deficiency was corrected, with compliance effective November 17, 2025.

  • A complaint claimed the community had a pest problem. The surveyor toured the building on entry and again throughout the three-day visit and saw no pests or bedbugs in resident rooms or common areas. 10-30-2025
  • Residents themselves told the surveyor they had not seen pests or bedbugs, and staff reported pest control comes monthly and as needed. 10-30-2025
  • A second complaint alleged residents were being financially exploited. Residents told the surveyor they had never been asked for or given money or gifts to staff members, and no deficiency was cited on that allegation. 10-30-2025
  • The surveyor watched staff helping residents with daily activities, and described them as doing so in a pleasant manner. 10-30-2025
Show all 6 findings
  • In its plan of correction the facility reported that after the kitchen sanitizer problem was found, nursing monitored the 17 residents who ate kitchen-prepared meals for gastrointestinal symptoms during routine daily observation and none were observed. This is the facility's own account rather than a surveyor finding. 10-30-2025
  • The state verified the kitchen problem was fixed, with compliance effective November 17, 2025. 10-30-2025

What the state also noted

  • One deficiency was cited for failing to clean and sanitize dishes used to prepare and serve resident meals; no sanitizer registered in the three-compartment sink or the dish machine, the sanitizer container was empty, the daily check had not been logged that day, and the facility could not produce a policy on sanitizer use and monitoring.
  • The state letter noted this deficiency represented the potential for more than minimal harm.
  • The corrective actions and the statement that no residents showed gastrointestinal symptoms come from the facility's own plan of correction, not from surveyor observation.
  • Only one survey from the last 24 months is in this record, so there is limited history to judge from.

Full inspection history

Every inspection Oklahoma has on record for this community. The rating is based on the last 2 years, so a community is not scored on something it corrected long ago — but the older record is here to read either way.

My help finding the right community costs your family nothing.

You never pay me — the community pays my fee, and only if someone I introduce moves in. And one person calls you back: me, not a call center.

  • The price above is not the price you’ll pay. Care-level charges, community fees, medication management and second-person fees all stack on top of that base rate. I’ll walk you through what this community really charges for someone in your loved one’s situation — and push to get the move-in fee and deposit reduced.
  • Whether they have a room, and would take your loved one. I’ll find out what’s actually open right now and whether they’d genuinely accept this level of care, before you spend a morning touring.
  • How it compares to the others nearby. I hold the same state record for every community in the metro. Their sales office will never tell you the place four miles away has a cleaner history.

Paul Swales, Senior Placement Advisor at Oklahoma Senior Placement

I’m Paul Swales, and this is what I do all day. A senior placement advisor is the person who finds the right community for your family and does the hard part for you. I’ve read the state health department’s inspection record on every community in this metro. I know which ones will genuinely take your loved one’s level of care, and I make the calls — so you’re not doing this alone at eleven at night on your phone.

It works like a buyer’s agent in real estate. Your family never pays me. The community pays my fee, and only if someone I introduce actually moves in. There’s no cost to your family and no obligation to keep using me.

The one catch is the order. They only pay my fee if I introduce you before you contact them yourself. Call them first and you’re a walk-in — I can’t step in as your advocate after that. It costs your family nothing either way. It only decides whether you have someone on your side.
