The Neighborhoods at Baptist Village of Okmulgee had one state inspection in the last two years, a three-day October 2025 relicensure survey that also investigated a complaint. The complaint alleged a pest problem and financial exploitation of residents; the surveyor toured the building repeatedly and saw no pests or bedbugs, residents said they had not seen any either, and residents said they had never been asked for or given money or gifts to staff - neither allegation resulted in a citation. The surveyor also noted staff assisting residents with daily activities in a pleasant manner. One deficiency was cited, in the kitchen: while dishes were being washed, no sanitizer registered in either the three-compartment sink or the dish machine, the dish machine's sanitizer container was empty, the required daily sanitizer check had not been done that day, and the facility could not produce a policy on sanitizer use and monitoring. In its plan of correction the facility said it replaced the sanitizer drum the same day, rewashed the affected cookware and utensils, retrained the dining team, wrote a new procedure and added ongoing monitoring, and that nursing watched the 17 residents who eat kitchen-prepared meals for gastrointestinal symptoms with none observed. The state confirmed the deficiency was corrected, with compliance effective November 17, 2025.
