The Parke's October 2025 inspection was a routine relicensure survey combined with a complaint alleging that a staff member had physically abused a resident. Inspectors sampled ten residents, observed staff interacting with residents, and reviewed incident reports and abuse training, and they did not cite the facility for abuse, neglect or mistreatment. They did cite one violation: the facility never sent the required Nurse Aide Registry notification form after that April 2025 allegation, even though it had removed the aide from resident care the same day, assessed the resident, and notified the family and physician. The paperwork gap was corrected and a state review on November 21, 2025 confirmed the facility was back in compliance.
