State inspectors visited The Renaissance of Ponca City from October 27 to 29, 2025 for a relicensure survey combined with three complaint investigations, and they cited three violations. The most serious was staffing: the center failed to provide sufficient staff to meet residents' care and evacuation needs for five residents identified as not capable of responding to an emergency without physical help from staff; 61 of 249 call-light pushes over a three-week stretch were answered more than 20 minutes after activation; and the executive director could not locate an evacuation policy. The other two citations were a kitchen worker not wearing a beard cover on three separate observations, and five resident rooms with in-room cameras or audio monitors that had no signage on or near the door, which the facility's own policy required. Resident accounts were mixed: one resident said they pushed their pendant and sometimes no one came at all, and another said they fell, pushed the pendant, could not get anyone to come, was helped up by a fellow resident, and waited about 30 minutes for staff — though that same resident said the staff themselves were good and simply too few on evenings and weekends. A family member interviewed on site said their relative received good care and that call lights were answered most of the time. Several other allegations inspectors looked into did not result in citations, including pest control, infection control, dignity and respect, and help with daily activities like bathing and dressing. The facility submitted a correction plan the state accepted, and on a February 2, 2026 off-site paper revisit the state found it back in substantial compliance effective January 26, 2026.
