What state inspectors found

Touchmark at Coffee Creek had one state inspection in this two-year window, in October 2025, which combined a regular licensing survey with two complaint investigations. Both complaints - one about safety and mental health services, one about staffing levels and fall prevention - were investigated in depth and the state found the community in compliance on both. The routine part of the survey did find two problems: the kitchen was not writing down food temperatures, and one medication aide hired in July had not completed her CPR training. Both were corrected, and a state follow-up review on November 17, 2025 confirmed the community was back in substantial compliance.

  • Someone complained that the community was not providing a safe environment or mental health services. Inspectors came out unannounced, looked at 11 residents' care, watched staff and residents, reviewed health charts, doctor's orders, assessments and care plans, and talked with residents and families - and concluded the community was following state rules. 10-16-2025
  • A second complaint alleged there were not enough staff and that fall-prevention steps were not being followed. Inspectors specifically watched staff-to-resident ratios and the help residents were getting with daily care, and reviewed incident reports, staffing schedules and actual time punches. They found the community in compliance on both points. 10-16-2025
  • The two problems inspectors did find were fixed quickly. The state accepted the community's correction plan and, on a follow-up review on November 17, 2025, confirmed the community was back in substantial compliance. 10-16-2025

  • This was not a clean survey. Two deficiencies were cited on 10/16/2025: food temperature documentation, and CPR training for 1 of 5 employees sampled.
  • On the food temperature citation, the executive chef stated "We don't have temperature logs," and staff said they check temperatures but do not record them. The state cited this as widespread — reaching across the community.
  • The CPR training gap involved one certified medication aide hired 07/05/25 whose file showed no CPR training; the administrator confirmed "it is not in [their] file because [they] don't have it."
  • Only one survey exists in the last 24 months, so there is no earlier inspection to compare against for trends over time.
  • The finding that the community was "in compliance with state regulations" applies to the specific complaint allegations investigated, not to the whole survey, which did cite the two deficiencies above.
