Town Village was inspected twice in the last two years, and the state cited problems both times — so this is not a spotless record. In March 2025 inspectors cited an unclean kitchen, a nebulizer stored improperly and a family not told about x-ray results, and found that only two staff were scheduled overnight when one resident's care plan called for three during transfers; the community corrected all of it and the state verified substantial compliance effective April 9, 2025. The May 2026 inspection was lighter. Surveyors handled three complaints: two alleging too little staff and residents needing more care than the community should provide, and a third alleging a resident had suspicious injuries of unknown origin. The staffing and level-of-care complaints were investigated in depth — surveyors watched residents during meals, medication passes and transfers, interviewed residents about feeling safe, and contacted family representatives — and the only citation was unlabeled food in a refrigerator and a cook without a beard net. The injury complaint was closed without further investigation because the resident named in it was found to live in independent living rather than the assisted living center, so nothing was concluded about it either way. Kitchen and food-service practices were cited at both inspections. If you tour, it is fair to ask how overnight staffing is set, how many caregivers assist residents who need help transferring, and how the community tracks and reports unexplained injuries.
