State inspectors came to Trinity Woods three times in the last two years, and two of those visits - a complaint investigation in September 2024 and the full relicensure inspection in October 2025 - ended with no deficiencies cited at all. On both 2024 visits the surveyor described residents relaxing in the common areas and in their rooms while staff attended to them and helped with daily care. Families should also know the August 2024 complaint visit did result in a citation: after a resident with dementia struck an aide, the aide struck the resident back on the shoulder, and the facility's own investigation substantiated the abuse. The aide was terminated, the resident was assessed and found uninjured, all campus staff were re-trained on abuse, and the state verified the correction at a September 2024 follow-up.
