This center had one state inspection in the last two years, in April 2025, and two deficiencies were cited. In the kitchen, opened food in the freezer and in both refrigerators was not labeled with open and use-by dates, and opened dry goods were not stored in containers with tight-fitting lids; the dietary manager told the surveyor she had never seen the kitchen policies and that the evening and weekend cook never labels and dates food. The center was also cited because no completed annual fire inspection had been done for 2025, and the previous fire inspection dated 05/17/2024 had listed multiple deficiencies with a 30-day correction timeline that were never re-checked by the fire marshal. OSDH classified both citations as representing the potential for more than minimal harm. The center's plan of correction was accepted, and a state follow-up on June 2, 2025 confirmed the deficiencies were corrected effective May 30, 2025. The April inspection also included a complaint investigation about supervision after a resident left the building in December 2024; inspectors reviewed records and interviewed residents, families and staff, and the citations that resulted were about the kitchen and the fire inspection, not about supervision or resident care.
