State inspectors spent five days at Village Lodge in November 2025, combining the routine licensure survey with three separate complaint investigations covering staffing, falls, restraints, sedation, medication handling and infection control. They observed residents at meals, during medication passes and in activities and recorded that residents were assisted with dignity and respect and were alert and not sedated, and none of the complaint concerns led to a citation.
