OSP RatingOSP FairHow we rate: the OSP Rating is built from this community’s Oklahoma State Department of Health (OSDH) survey history — weighing how recent, how frequent, and how serious the state’s inspection findings are. (Assisted living has no federal star rating, so we base it on the state’s public survey record.)

Faira solid, generally acceptable record, with a few state findings worth a quick question.

📞Often nothing to worry about. The state’s findings are summarized below — call Paul if you want help judging whether any of them would matter for your loved one.

Based on Oklahoma state (OSDH) survey history — assisted living and memory care have no federal star rating.

What state inspectors found

1 inspection in the last two years · 1 with no deficiencies cited

West Wind's most recent state inspection, in November 2025, was a full relicensure survey combined with two complaint investigations - one alleging abuse and one about families not being able to reach staff by phone - and it closed with no deficiencies cited at all. The surveyor wrote that staff were seen assisting residents and interacting with them in a dignified and respectful manner, and that residents were observed at meals, during medication passes, and during activities. Investigators also reviewed care plans, physician orders, assessments, incident reports, employee files and staff training records without finding a problem. There were 47 residents living there at the time of the visit.

  • A separate complaint about families not being able to reach staff by phone was also investigated - including reviewing the facility's emails about phone service and repairs and watching phone use in the building - and no deficiency was cited. 11-10-2025
  • Investigators went through resident records, physician orders, care plans, assessments, incident reports, employee files and staff training records without citing a problem in any of them. 11-10-2025

What the state also noted

  • Only one survey falls within the last 24 months, so this picture rests on a single inspection.
  • The site's own rating note references an April 2024 citation for failing to timely report a resident-on-resident sexual-abuse incident. That citation predates this 24-month window and the underlying survey document is not in this record, so nothing here confirms or describes it. Families may want to ask the facility or OSDH about it directly.
  • 'No deficiencies cited' means the surveyor found no rule violations during that visit. It is not a guarantee about day-to-day care at other times.
