What state inspectors found

Westbrook Gardens had one state survey in the last two years, on December 10, 2025, and it produced three citations, so the record is not spotless. A complaint that residents were not being kept free from abuse was investigated that day through resident interviews, record reviews and a check of incident, state-reportable and grievance files, and no abuse-related deficiency was cited. During the same visit the surveyor saw staff helping residents and residents moving about the halls and talking with one another. The three citations were: no documented fire marshal inspection since 08/01/22, with none in 2024 or 2025; a certified medication aide observed giving a resident a 15,000 mg cranberry capsule when the physician's order was 450 mg — roughly 33 times the ordered dose, in a resident whose diagnoses included a urinary tract infection; and an operating license renewal not filed until 12/02/25, after the license had expired on 10/19/25. The facility's first plan of correction was rejected as unacceptable, a revised plan was accepted on 01/09/26, and an offsite paper revisit on 02/04/26 found the facility back in substantial compliance. Worth asking on a tour: whether the fire marshal inspection was actually completed, and how family-supplied over-the-counter medications are checked against physician orders.
