Zarrow Pointe's March 2025 licensure survey resulted in one citation, and it was a fire-safety one: the center could not show that the required annual fire marshal inspection had been completed in 2024. The record notes the last completed fire marshal inspection was October 6, 2023, so the building had gone roughly 17 months without one. OSDH scored the citation at scope level F, the broadest scope, and its letter states the deficiency carried the potential for more than minimal harm. The facility obtained a Tulsa Fire Department licensing inspection on March 13, 2025, one week after the survey, and the state closed the citation on an offsite/paper revisit in April 2025. No complaint investigations appear in the last two years of records.
